David A. Bellows, MD, FACS

Dr. Bellows attended the University of Wisconsin and the University of Illinois where he received his bachelor’s degree in physiology. He continued his studies at the Chicago Medical School where he graduated in the top ten of his class and was awarded the prize for Outstanding Performance in Surgery. In that same year he was chosen for membership in Alpha-Omega-Alpha, the Honorary Medical Society.
Dr. Bellows continued his training at the University of Pennsylvania where he completed his residency in eye surgery at the Scheie Eye Institute. He is certified by the American Board of Ophthalmology.
Dr. Bellows is a Diplomate of the American Board of Ophthalmology and a Fellow of the American College of Surgeons. He is a member of the American Academy of Ophthalmology, the New Hampshire Society of Eye Physicians and Surgeons and the New Hampshire Medical Society, the North American Neuro-Ophthalmology Society and the European Neuro-Ophthalmology Society.
In addition to his work at the Eye Center, Dr. Bellows is a member of the faculty at the Tufts/New England Medical Center in Boston. He has served four terms as chairman of the Department of Ophthalmology at Catholic Medical Center and he currently serves as co-chairman of the laser committee at the Elliot Hospital.
Dr. Bellows is a member of an international group of neuro-ophthalmologists involved in research on Ischemic Optic Neuropathy. He has also participated in research that led to the approval of intraocular lens implantation in children and the use of the excimer laser for laser vision correction.
Dr. Bellows’ primary areas of interest are glaucoma and the neurological diseases that affect the visual system (neuro-ophthalmology).

Terry B. Goodman, OD

Dr. Goodman received his Bachelor of Arts degree with high honors from the University of New Hampshire in 1972, majoring in zoology and minoring in psychology. While there he was elected to a number of honor societies including Phi Beta Kappa, Alpha Epsilon Delta, Phi Sigma and Psi Chi. In 1971 he received an appointment in the Undergraduate Research Fellowship Program. Dr. Goodman also served as an instructor in psychology in 1972.
In 1978 Dr. Goodman received his Doctor of Optometry degree (OD), with honors, from the New England College of Optometry. He was awarded the Dexter Memorial Scholarship in 1975, 1976 and 1977. In 1975 he was elected to Beta Sigma Kappa, the Optometric Honor Society. He also served as editor of his class yearbook.
Dr. Goodman was in private practice before joining the Medical Eye Center in 1987.
Dr. Goodman has lectured for the New York College of Optometry, the Elliot Hospital Medical Staff and Catholic Medical Center.
In 1997 Dr. Goodman was chosen to receive the Derryfield School’s Distinguished Alumni Award. In that same year he was awarded the New Hampshire Association of the Blind’s Vision of New Hampshire Award.
Dr. Goodman is a member of the American Optometric Association and the New Hampshire Optometric Association.
Dr. Goodman is active in the Lions Clubs International. He has served as District Governor and Council Chairman for the State of New Hampshire. He lectures around the country on a variety of topics relating to Lionism.
Dr. Goodman and his family live in Bedford, New Hampshire. 
George J. Shaker, MD, FACS

Dr. Shaker is a graduate of Amherst College in Amherst, Massachusetts. He continued his studies in Medicine at the University of Rochester School of Medicine. He graduated with Honors and was awarded the Doran Stevens Prize for excellence in patient care. During medical school, Dr. Shaker spent one year as a fellow in the Department of Ophthalmology studying factors which affect corneal wound healing as well as the treatment of keratitis sicca (dry eye), work which resulted in the publication of several journal articles and the honor of Distinction in Research.
Dr. Shaker completed his training in Ophthalmology and eye surgery at the University of Chicago. He was chosen to serve as the department’s Chief Resident. Dr. Shaker became certified by the American Board of Ophthalmology in 1995.
Dr. Shaker is a Diplomate of the American Board of Ophthalmology and a Fellow of the American College of Surgeons. He is a member of the American Academy of Ophthalmology, the American Society of Cataract and Refractive Surgery, the New Hampshire Medical Society, the Hillsborough County Medical Society and the New Hampshire Society of Eye Physicians and Surgeons.
Dr. Shaker is on staff and performs surgery at the Elliot Hospital and the VA hospital in Manchester. He is also on staff at Catholic Medical Center. He serves on the Credentials Committee at the Elliot Hospital and on the Advisory Board of the Elliot One Day Surgery Center.
He has been selected (by fellow physicians) as “The Top Ophthalmologist” in the State of New Hampshire (New Hampshire Magazine) in 2006, 2008 and 2009.
Dr. Shaker’s primary area of interest is the surgical treatment of eye conditions. He has a special interest in cataract surgery, including the use of special implants that correct for distance and near vision and astigmatism, as well as glaucoma surgery.
Dr. Shaker lives in Manchester with his wife and two daughters. While both girls are now away for college, happily they do make their way home for holidays and vacations!

Steven M. Siegal, OD

Dr. Siegal attended the University of Massachusetts at Amherst. During his undergraduate years, Dr. Siegal was actively involved in Neighborhood Youth Corps, counseling and training underprivileged youth. While attending the University of Massachusetts, Dr. Siegal received accelerated admission to The New England College of Optometry in Boston, MA.
Dr. Siegal received both his Bachelor of Science and Doctor of Optometry degrees from The New England College of Optometry in 1982. He was associate editor of the yearbook and a member of the student council. He completed his internships at the Brighton Marine Public Health Hospital in Brighton, MA and the Great Brook Valley Health Center in Worcester, MA.
Dr. Siegal has been practicing optometry for many years, during which time he has received numerous awards for patient care and satisfaction. He has been a past recipient of the Patient Satisfaction Award, in recognition of his commitment, effort, and excellence of care in the northeast region.
Dr.Siegal joined the Medical Eye Center in 2001, after practicing in Manchester since 1993.
Dr. Siegal is a TPA/g certified optometrist, authorized to treat glaucoma by the New Hampshire Board of Registration in Optometry.
Dr. Siegal was a charter member of the American Optometric Association Contact Lens Section. He is a member of the New Hampshire Optometric Association and the American Optometric Association.
Dr. Siegal is licensed to practice optometry in New Hampshire, Massachusetts, and Connecticut.
Dr. Siegal and his family live in Chelmsford, Ma.
Jonathan D. Casciano, MD

Dr. Casciano was born and raised in Little Rock, Arkansas where he was active in the youth symphony orchestra. In 1996 he received a baccalaureate degree in History and a masters degree in American Civilization from Brandeis University. He met his future wife, Shelley, in the orchestra when they were standpartners in the violin section.
After graduation from Brandeis University Dr. Casciano worked in the Biotech industry which “sparked” his interest in medicine. 
He completed his premedical training at the University of Massachusetts and returned to Arkansas where he received his medical degree in 2005. He completed his internship in Internal Medicine and residency training in ophthalmology at the University of Arkansas. In 2009 he received the “Resident of the Year” award.
Dr. Casciano completed his fellowship training in diseases of the Cornea and External Eye at the University of Michigan Kellogg Eye Center prior to joining the Medical Eye Center. He is certified by the American Board of Ophthalmology and is a member of the American Academy of Ophthalmology, the Cornea Society, the American Society of Cataract and Refractive Surgeons, the New Hampshire Medical Society and the New Hampshire Society of Eye Physicians and Surgeons.
Ahad A. Fazelat, MD, MPH

Dr. Fazelat completed his two-year fellowship in retinal diseases and surgery at the Massachusetts Eye and Ear Infirmary in June of 2011. During that time, he also was a research fellow at the Schepens Eye Research Institute of Harvard Medical School, where he remains involved in several studies related to the treatment of retinal disease. He did his residency in Ophthalmology at the Albert Einstein College of Medicine in New York City, where he was also chosen to be the chief resident. Prior to medical school, Dr. Fazelat, who grew up in Missouri, obtained his undergraduate degree in Biology at Truman State University and the University of Missouri and a masters degree in public health at Boston University.
Dr. Fazelat is certified by the American Board of Ophthalmology and treats patients with the full spectrum of diseases of the retina and vitreous. He is a skilled clinician and surgeon and has the type of personality that is certain to put patients at ease when discussing their eye conditions.
Dr. Fazelat and his family have recently relocated to Bedford, NH. His wife, an emergency room physician, grew up in New Hampshire and is very happy to have “come home.” The Fazelats have two children, who no doubt eat very well, as Dr. Fazelat’s passion other than ophthalmology is cooking!
Jason A. Hall, MD
Dr. Jason Hall completed his glaucoma fellowship at the Duke University Eye Center in Durham, North Carolina. He did his residency in ophthalmology at the Indiana University School of Medicine in Indianapolis, during which time he made several presentations at the Association for Research in Vision and Ophthalmology annual meeting and published a paper in the professional journal “Ophthalmology”, all concerning various aspects of the medical and surgical treatment of glaucoma.
Dr. Hall earned his medical degree at University of Toledo School of Medicine finishing at the top of his class. Upon graduation he received the Glidden L. Brooks Award, which is presented annually to the graduating student who is distinguishable by superiority in all phases of the curriculum. He was also inducted into the Arnold P. Gold Foundation Humanism Honor Society and the Alpha Omega Alpha Honor Medical Society. Prior to medical school, Dr. Hall, who grew up outside of Albany, NY, obtained his undergraduate degree in Biology with Highest Honors at Cedarville University in Ohio.
Dr. Hall is a skilled clinician and surgeon certified by the American Board of Ophthalmology. He practices consultative and surgical ophthalmology with a specialization in state-of-the-art glaucoma surgery.
He and his wife and three children live in the Manchester area. Dr. Hall enjoys spending his free time with his family and is an avid tennis player. He also looks forward to participating in international medical missions.
David A. Weinberg, MD, FACS

Dr. Weinberg is an ophthalmologist with fellowship training in the subspecialties of oculofacial plastic surgery and neuro-ophthalmology. Dr. Weinberg’s practice encompasses all aspects of oculofacial plastic surgery, including that of the eyelid, the tear (lacrimal) duct, and the orbit. Cosmetic procedures offered include botulinum toxin (BOTOX®) and filler injections, blepharoplasty, and forehead and mid-face lifts.
After the completion of a six-year biomedical program at Rensselaer Polytechnic Institute and Albany Medical College, Dr. Weinberg obtained his residency training in ophthalmology at the University of Cincinnati Medical Center. This was followed by fellowships in neuro-ophthalmology at Michigan State University and Wills Eye Hospital in Philadelphia. Dr. Weinberg then completed a fellowship program in oculofacial plastic surgery at the Jules Stein Eye Institute of UCLA in Los Angeles, California.
After finishing his training, Dr. Weinberg was chief of the ophthalmic plastic surgery and neuro-ophthalmology services and fellowship director at Fletcher Allen Health Care and the University of Vermont for ten years, prior to a recent two year associate professorship at the University of Utah.
Dr. Weinberg is certified by the American Board of Ophthalmology and is a fellow of the American Academy of Ophthalmology, the American Society of Ophthalmic Plastic and Reconstructive Surgery, the North American Neuro-Ophthalmology Society and the American College of Surgeons. He has been President of the New England Oculoplastic Society since 1996. |
